We are accepting new patients!

We are using a new online service called Register with a Practice that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need an NHS number to register online. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

If your address is outside our practice boundary your application will be declined; but you will be advised of other practices in your area available to join.

Paper registration forms are still available at our reception if you need one.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

Please note that all patients have been allocated a named accountable GP. While you may see any GP you like for appointments, everyone has a named accountable GP who they are registered with who is responsible for their over all care. This will be one of our partner GPs- Dr Athan Simopoulos, Dr Katie Ariaraj or Dr Jonathon Minns. Please ask at reception to find out who is yours.

Accessing someone else's information

As a parent, family member or carer, you may be able to manage online services for another person by switching to their profile. This is also sometimes called having a linked profile or having proxy access. It needs to be set up by the surgery where the patient is registered. You can then switch profiles using the NHS App and order medication on their behalf and view their upcoming appointments.

In order to apply for proxy online access you will need to complete this application form PatientOnline Proxy consent form and pass it to the practice. The form will need to be signed by the patient and those applying to have proxy access to their account. In the case of a child under 16, the parent can complete the form without the child’s signature, but once the child turns 16 the proxy access will be removed and the child would be expected to manage online services for themselves. If the patient does not have capacity to complete the form then someone with Lasting Power of Attorney can complete it on their behalf.
