We have recently conducted an audit and found that a number of patients are living outside of our practice boundary. Some patients are also living in another health authority which means we are unable to provide care for them at all. If you have received a text message from us asking to re-register because you are out of area, this is because we have a new partnership who think it is unsafe to have patients on our list who we are unable to conduct home visits for and it is no longer viable for you to remain on our list. We work as a Primary Care Network with integrated teams such as the community nurses on a local basis and care can become disjointed if you are under another area’s team. Patients who live out of area are statistically more likely to miss GP appointments and attend out of hours clinics because it is closer to home than the surgery is. We kindly ask that you register at another surgery which is in your locality as soon as possible.
